Nursing Care Plans
Written the way a care plan is actually graded and the way the exam actually tests it. Assessment, priority diagnoses, measurable goals, interventions with the rationale attached to each one, evaluation, and a short note naming what the item is really asking.
159 plans across 13 body systems
Most care plan collections give you a list of interventions and leave you to guess why each one is there. That is the part the exam tests and the part a clinical instructor asks about, so every intervention here carries its rationale and every plan closes by naming the reasoning the item is built on.
Each plan is one page and prints as a clean handout, so it works on a screen the night before an exam and on paper in a clinical binder. Plans are grouped by body system rather than by client need, which is how you study when you already know the topic you are weak in. Choose a system to see its plans.
Cardiovascular
Chest pain, pump failure, rhythm, and clot. These plans share one tested habit: deciding what cannot wait.
15 plansRespiratory
Oxygenation, ventilation, and infection control. Watch for deterioration that presents as a patient who has gone quiet.
14 plansNeurologic
Level of consciousness first, then focal findings. The tested skill is noticing a change early enough for it to matter.
14 plansGastrointestinal
Bleeding, obstruction, and liver failure. Watch volume status and mental status together rather than separately.
12 plansRenal and Genitourinary
Access protection, potassium, and fluid balance carried across every shift.
12 plansEndocrine
Sequence, teaching accuracy, and the ordinary stressor that becomes an emergency. Most errors here are order of operations errors.
12 plansHematologic and Oncologic
Bleeding risk, infection risk, and transfusion safety. Small findings carry large consequences here.
10 plansPerioperative Care
Before, during, and after. Most tested errors are handoff errors and missed early complications.
13 plansFluid, Electrolyte, and Acid Base
Numbers with a nursing action attached. The item turns on what you do about the value, not on the value itself.
12 plansMusculoskeletal
Neurovascular checks, mobility, and the complication that presents as pain out of proportion.
12 plansImmune and Infectious
Transmission based precautions, immunosuppression, and recognizing sepsis before the blood pressure falls.
10 plansIntegumentary and Burns
Airway, fluid resuscitation, and wound care in that order. Timing drives every decision.
8 plansSensory
Vision and hearing loss, safety, and the postoperative restrictions patients most often break.
Opening a plan
Every title here is a real plan. The full body of each one, including the interventions and rationales, is part of a NurseStudy Prep membership. Log in to open them, or see what a membership includes.
How the library is growing
Every body system is live and plans are still being added inside each one. The cheat sheet library and the flashcard decks cover the same content in a faster format.
